In this per diem role, you will conduct comprehensive home health risk assessments for Medicare Advantage plan members throughout Preble County, Ohioincluding Eaton, Camden, Lewisburg, and West Alexandria. Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling allows you to tailor your hours
- Minimum weekly or bi-weekly commitment as determined by your availability
Key Responsibilities:
- Perform detailed annual home health risk assessments for Medicare Advantage plan members
- Collect and evaluate comprehensive medical, medication, and healthcare history
- Provide education to members regarding their health conditions and current treatments
- Identify potential care gaps and make clear recommendations for follow-up
- Visits focus on assessmentno need to create treatment plans or prescribe medications
Qualifications:
- Unencumbered Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- A minimum of 6 months of Nurse Practitioner experience preferred
