Step into a dynamic per diem role providing crucial in-home health risk assessments for Medicare Advantage plan members across Preble County. This position offers unmatched flexibility, allowing you to create your own schedule with shifts available any day of the week. Assignments may include home visits in communities such as Eaton, Camden, Lewisburg, and West Alexandria, with a focus on annual health assessments or post-discharge visits.
Schedule:
- Flexible shifts offered Monday through Sunday
- Design your own work schedule
- Commit to a preferred number of weekly or bi-weekly hours
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members
- Review and document prior/current medical history, ongoing conditions, and medication regimens
- Provide individualized education to patients on health conditions and prescribed medications
- Identify care gaps and recommend actionable steps to resolve them
- Note: This role does not involve developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current Basic Life Support (BLS/CPR) certification
- Board Certification from AANP or ANCC
- MSN from an accredited nursing program
- A minimum of 6 months' experience as a practicing NP (preferred)
