Schedule:

• Flexible shifts available any day, Monday through Sunday
• Choose your own schedule and locations across Ripley County, including Batesville, Versailles, and Osgood
• Commitment options for weekly or bi-weekly hours to suit your needs

Key Responsibilities:

• Perform comprehensive annual health risk assessments in home settings, averaging 45 minutes each
• Review and assess medical history, current conditions, and medications for Medicare Advantage members
• Deliver patient education regarding health conditions and medication management
• Identify care gaps, offering recommendations to promote patient health and close those gaps
• Maintain a focus on assessment and education; no need to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Indiana
• Current CPR certification (BLS)
• Board certification through AANP or ANCC
• A minimum of 6 months of recent NP experience preferred
• Masters degree in Nursing (MSN) from an accredited institution
