Deliver personalized care in a home setting as an experienced and compassionate Nurse Practitioner. This per diem opportunity involves conducting home health risk assessments for Medicare Advantage plan members in Fayette County, INincluding locations such as Connersville, Glenwood, and Bentonville. Each evaluation takes approximately 45 minutes and may include annual health assessments or post-discharge visits, providing essential care insights without the responsibility of treatment planning or prescribing medications.
Schedule:
- Flexible shifts available between Monday and Sunday
- Choose your workload by setting your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual risk assessments for Medicare Advantage plan members in home health settings
- Review medical histories, current conditions, and medication profiles
- Offer patient education on chronic conditions and medications
- Identify care gaps and recommend next steps for improved outcomes
- Complete thorough documentation; no prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner license in Indiana
- Valid CPR certification (BLS)
- Board certified through AANP or ANCC
- 6 months or more of recent Nurse Practitioner experience preferred
- MSN from an accredited nursing program
