An exciting per diem opportunity is available for a Nurse Practitioner skilled in home health assessments. This role focuses on conducting home health risk evaluations for Medicare Advantage members throughout Fayette County, IN, including Connersville, Glenwood, and Bentonville. Each visit typically lasts about 45 minutes and may involve either annual health assessments or post-discharge check-ins. Practitioners may be scheduled in various locations within the county for each shift.
Schedule:
- Flexible shifts available from Monday through Sunday
- Option to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan participants
- Review prior and current medical history, existing conditions, and medication usage
- Educate patients about their medical conditions and medications
- Identify and document any gaps in care, offering recommendations as appropriate
- No requirement for creating treatment plans or prescribing medications
Desired Qualifications:
- Active Indiana Nurse Practitioner license
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- Minimum of 6 months of Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
