Step into a dynamic and impactful role providing home health risk assessments for Medicare Advantage plan members across Tippecanoe County, INincluding Lafayette, West Lafayette, and Battle Ground. As a Nurse Practitioner specializing in home health, you will be responsible for conducting comprehensive assessments during annual health checkups or post-discharge visits, averaging about 45 minutes per evaluation. Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule and workdays
• Minimum commitment required on a weekly or bi-weekly basis

Key Responsibilities:

• Perform thorough home health risk assessments for Medicare Advantage members
• Review prior and current medical history, ongoing medical conditions, and medications
• Educate patients on health conditions and safe medication usage
• Identify healthcare gaps and make clear recommendations
• Focus exclusively on assessmentno treatment planning or prescribing needed

Qualifications:

• Active Nurse Practitioner license in Indiana
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred
