Step into a dynamic role as a Nurse Practitioner conducting home health risk assessments throughout Tippecanoe County, IN, serving Medicare Advantage plan members in areas such as Lafayette, West Lafayette, and Battle Ground. In this per diem opportunity, youll play a crucial part in assessing patient health in their own homes, improving care outcomes, and making a meaningful difference.

Schedule:

  • Flexible shifts available any day of the week, Monday through Sunday
  • Create your own schedule based on your availability
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments, typically taking around 45 minutes each
  • Review and evaluate prior and current medical history, existing conditions, and medication regimens
  • Educate patients on their conditions and prescribed medications
  • Detect care gaps and offer recommendations to address them
  • Visits vary and may include annual health assessments or post-discharge evaluations
  • No requirement to prescribe medications or create treatment plans

Qualifications:

  • Valid Nurse Practitioner license in Indiana
  • Current BLS/CPR certification
  • Board certification (AANP or ANCC)
  • Minimum of 6 months recent nurse practitioner experience preferred
  • MSN from an accredited nursing program
