Professional opportunity available for skilled Nurse Practitioners with a background in home health to support patient care in the Brooklyn, NY area. This per diem role focuses on conducting risk assessments for Medicare Advantage members in their homes, with each appointment lasting approximately 45 minutes. Assignments may include annual health evaluations or post-discharge assessments and will be scheduled throughout various locations in New York, NY.
Schedule:
- Flexible shifts offered Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Assess prior and current medical conditions, medical history, and medication usage
- Educate patients on their medical conditions and prescribed medications
- Identify care gaps and recommend actions to address them
- No requirement for treatment planning or prescribing
Qualifications:
- Active Nurse Practitioner license in New York
- Valid BLS (CPR) certification
- Board Certification via AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- A minimum of six months of current Nurse Practitioner experience preferred
