Job Description

Seeking a nurse practitioner with experience in home health for a unique per diem opportunity in Brooklyn, NY. This position focuses on conducting home health risk assessments for Medicare Advantage plan members. Each assessment visit typically lasts about 45 minutes and may involve an annual health check or a post-discharge evaluation. The scheduling is highly flexible, with the possibility to create your own work hours and select shifts between Monday and Sunday to accommodate personal and professional commitments. Assignment locations may vary across Brooklyn during a given shift.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage plan members
  • Review and evaluate past and current medical history, conditions, and medication usage
  • Educate patients on medical conditions and prescription adherence
  • Identify care gaps and provide recommendations to address them
  • No treatment planning or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in New York
  • Current BLS/CPR certification
  • Board certified through AANP or ANCC
  • Minimum of 6 months NP experience preferred
  • MSN from an accredited nursing program

This role offers $80-$90 per evaluation, as well as a mileage stipend and compensation for administrative time. The focus remains on assessment and patient educationno prescribing, charting treatment plans, or on-call responsibilities.
