Job Description
Make a meaningful impact by delivering home-based care to Medicare Advantage members as a Nurse Practitioner in the Bronx, NY area. This per diem opportunity focuses on performing home health risk assessments, supporting patients to proactively manage their health and identify potential care needsall without requiring you to create treatment plans or prescribe medications.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule based on personal availability
- Choose between weekly or bi-weekly minimum hour commitments
Key Responsibilities:
- Conduct comprehensive annual risk assessments in members homes
- Review patients prior and current medical histories, existing conditions, and medication usage
- Provide education on ongoing medical conditions and medication regimens to foster self-care
- Identify gaps in care and make clear recommendations to close those gaps
- No requirements for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner (NP) license in the state of NY
- Current Basic Life Support (BLS) CPR certification
- Board certified through the AANP or ANCC
- Minimum of six months of current NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
