Job Description

Take on an impactful role as a Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members. This per diem opportunity offers flexibility and autonomy, allowing you to create your own schedule and serve patients in their homes. Each assessment generally takes around 45 minutes and may include annual health evaluations or post-discharge visits.

Schedule:

  • Flexible shifts offered between Monday and Sunday
  • Create your own schedule: plan shifts around your availability
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage members
  • Evaluate prior and current medical history, medical conditions, and medication usage
  • Educate patients on health conditions and proper medication management
  • Identify and address gaps in care, making effective recommendations
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board Certification (AANP or ANCC)
  • Current CPR certification (BLS)
  • Minimum of 6 months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited program

Whats Offered: This position pays between $80 and $90 per evaluation. Additional compensation is provided for mileage and administrative time, making this a rewarding and flexible role.
