This per diem opportunity offers the flexibility to create your own schedule, with shifts available between Monday and Sunday. Each home visit takes approximately 45 minutes and may encompass an annual health assessment or a post-discharge evaluation for Medicare Advantage plan members within the Bronx, NY area. Assignments may vary by shift, providing dynamic and rewarding professional experiences.
Schedule:
- Flexible shifts throughout the week (MondaySunday)
- Craft your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review current and past medical history, ongoing medical conditions, and medication use
- Educate patients on their health status and medication regimens
- Identify care gaps and provide recommendations to address them
- Note: No treatment plans need to be created, and prescribing medications is not part of this role
Qualifications:
- Nurse Practitioner license valid in NY State
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- At least 6 months of active Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
For each evaluation, receive compensation between $80 and $90, plus a mileage stipend and administrative time included.
