Schedule:

  • Flexible shifts offered Monday through Sunday
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members.
  • Review and evaluate both previous and current medical history, conditions, and medication usage.
  • Deliver patient education related to medical conditions and medications in a clear, supportive manner.
  • Identify care gaps and recommend strategies for improved health outcomes.
  • Focus solely on assessmentsno prescription writing or treatment plan creation required.
  • Each assessment typically takes approximately 45 minutes; visits may include annual evaluations or post-discharge check-ins.

Qualifications:

  • Active Nurse Practitioner (NP) License in New York
  • Board Certification from AANP or ANCC
  • Current CPR (BLS) certification
  • At least 6 months of current experience as an NP (preferred)
  • Master of Science in Nursing (MSN) from an accredited program
