Job Description
Deliver essential care directly to patients homes by conducting health risk assessments for Medicare Advantage members. This per diem opportunity is ideal for Nurse Practitioners with home health experience who appreciate autonomy and flexible scheduling. Responsibilities focus on patient evaluation and health education rather than ongoing treatment or medication management.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients current and prior medical history, health conditions, and medication usage
- Educate patients on their medical conditions and medications, ensuring understanding and adherence
- Identify and address gaps in care by recommending strategies for resolution
- Visits are typically 45 minutes in length and include both annual assessments and post-discharge evaluations
- No treatment planning or prescribing responsibilities
Qualifications:
- Active Nurse Practitioner license in New York
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- Minimum of 6 months of nurse practitioner experience preferred
- Previous home health experience is a plus
Compensation for this role includes a payment of $80$90 per evaluation, along with a mileage stipend and compensation for administrative time.
