Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Choose your own schedule
- Commit to a minimum number of hours weekly or bi-weekly
Responsibilities:
- Conduct annual and post-discharge home health risk assessments, typically 45 minutes each, in patients residences throughout Bronx, NY
- Review and evaluate patients medical histories, conditions, and medication regimens
- Educate patients on their health conditions and medications to promote better health outcomes
- Detect care gaps and provide actionable recommendations to support continuity of care
- Focus exclusively on assessmentsno prescribing or creation of treatment plans required
Qualifications:
- Active Nurse Practitioner license in NY
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner (preferred)
- MSN degree from an accredited nursing institution
This position offers compensation of $80-$90 per completed evaluation, along with a mileage stipend and consideration for administrative time.
