Put your advanced assessment skills to work as a Nurse Practitioner in a flexible, per diem role focused on home health risk assessments. In this position, youll visit Medicare Advantage plan members throughout Westchester Countyincluding New Rochelle, Mount Vernon, White Plains, Yonkers, and nearby areasto conduct thorough, patient-centered evaluations in the home setting. Each assessment typically takes about 45 minutes and may be annual or post-discharge in nature.

Schedule

• Flexible shifts available Monday through Sunday
• Freedom to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities

• Perform annual home health risk assessments for a Medicare Advantage health plan
• Review and evaluate medical history, current conditions, and all medications
• Educate members about medical conditions and medication regimens
• Identify gaps in care and make recommendations for resolution
• No requirement for treatment plan creation or prescribing medications

Qualifications

• Active Nurse Practitioner license in New York
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing school
• 6 months or more of current nurse practitioner experience preferred
