Take on an impactful per diem opportunity providing home health risk assessments for Medicare Advantage plan members. This role centers around the Bronx and various communities in Westchester County, including Yonkers, New Rochelle, Mount Vernon, and White Plains. As a Nurse Practitioner specializing in home health, you will play a critical part in ensuring patients have comprehensive evaluations and receive important health education within the comfort of their own homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Minimum hourly commitment weekly or bi-weekly

Key Responsibilities:

• Conduct annual risk assessments for Medicare Advantage members through home visits
• Review prior and current medical history, including all medical conditions and medications
• Educate patients on their health conditions, medications, and recommended next steps
• Identify any gaps in care, and communicate actionable recommendations
• Assessment-only role: No treatment planning or medication prescription is required

Qualifications:

• Active Nurse Practitioner license in the state of New York
• Current CPR certification (BLS)
• Board certification (AANP or ANCC)
• MSN degree from an accredited nursing program
• At least 6 months of current NP experience in a clinical setting preferred
