Advance your career as a nurse practitioner specializing in home health assessments. This per diem position offers the flexibility to create your own schedule while making a meaningful impact on Medicare Advantage plan members in Wagoner County, OKincluding Coweta, Wagoner, and Porter. You will conduct comprehensive risk assessments in patients homes, with each visit lasting approximately 45 minutes. Assessments may include annual health evaluations and post-discharge visits, supporting high-quality care and patient education.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling; set your preferred hours
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members
• Review and document patients' prior/current medical history, health conditions, and medications
• Educate patients on various health topics, including medical conditions and medication management
• Identify gaps in care and offer recommendations for addressing them
• No prescribing or creation of treatment plans required

Qualifications:

• Licensed Nurse Practitioner in Oklahoma
• Board Certification via AANP or ANCC
• Graduated with an MSN from an accredited nursing program
• Current CPR certification (BLS required)
• At least 6 months of nurse practitioner experience preferred
