Deliver meaningful care in a flexible per diem role designed for a skilled Nurse Practitioner experienced in home health. This position involves conducting home health risk assessments for Medicare Advantage plan members across Green County, including Monroe, Brodhead, and New Glarus. Shifts are highly flexiblechoose your own schedule, with opportunities for both minimum weekly or bi-weekly commitments, Monday through Sunday.
Schedule:
- Flexible shifts available any day of the week
- Create your preferred schedule
- Minimum hourly commitments on a weekly or bi-weekly basis
Your Key Responsibilities:
- Complete annual home health risk assessments in members' homes
- Conduct thorough evaluations of prior and current medical history, medical conditions, and prescriptions
- Offer education regarding diagnoses and medications to empower patients
- Identify and report gaps in care, suggesting appropriate recommendations
- No responsibility for treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Wisconsin
- Board Certification from the AANP or ANCC
- Current BLS/CPR certification
- Master of Science in Nursing (MSN) from an accredited institution
- Six months of recent NP experience preferred
