Job Description

Make a real difference in patient outcomes by providing essential home health risk assessments for Medicare Advantage members throughout Middlesex County, Massachusetts. This per diem position offers unmatched flexibility, giving Nurse Practitioners autonomy to design their own work schedules and achieve true work-life balance. Youll conduct thorough home visits in a range of locationsincluding Cambridge, Lowell, and Newtonto support both annual health and post-discharge assessments.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members
  • Evaluate medical and medication histories, as well as current health conditions
  • Educate patients about health conditions and prescribed medications
  • Identify gaps in care and offer actionable recommendations for follow-up
  • No requirement to prescribe medications or create formal treatment plans

Qualifications:

  • Active Nurse Practitioner license in Massachusetts
  • MSN from an accredited nursing program
  • Board Certification (AANP or ANCC)
  • Current CPR (BLS) certification
  • At least 6 months of recent Nurse Practitioner experience preferred

This position offers a pay of $80$90 for each evaluation, with mileage stipends and administrative time factored in.
