Job Description

Experienced nurse practitioners are needed to perform in-home health risk assessments for Medicare Advantage plan members across Fulton County, NY, including areas such as Gloversville, Johnstown, and Broadalbin. This per diem role offers exceptional flexibility, allowing you to create your own schedule with minimum weekly or bi-weekly hour commitments and flexible shifts available any day between Monday and Sunday.

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members during home visits
  • Gather and document patients' medical and medication history and evaluate current health status
  • Educate members about their medical conditions and prescribed medications
  • Identify gaps in care and make recommendations to support improved health outcomes
  • Visits last approximately 45 minutes; may include annual evaluations or post-discharge check-ins
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Current BLS/CPR certification
  • At least 6 months of clinical NP experience preferred
  • Strong assessment and communication skills
  • Comfortable providing care independently in patient homes

Those selected will earn $80$90 per evaluation, as well as a mileage stipend and compensation for administrative time.
