Step into an impactful per diem role focused on delivering quality care through home health risk assessments for Medicare Advantage plan members. As a Nurse Practitioner specializing in Home Health, you'll conduct visits in Gloversville, Johnstown, Broadalbin, and other areas within Fulton County, NY. Each assessment typically takes about 45 minutes and could range from annual health checkups to post-discharge evaluations, depending on the patients needs.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Design your own schedule to suit your availability
  • Minimum hourly commitment, weekly or bi-weekly

Key Responsibilities:

  • Complete comprehensive home health risk assessments for the Medicare Advantage population
  • Review patients' medical history, current health conditions, and medication lists
  • Educate patients on their diagnoses and medications to promote understanding and compliance
  • Identify gaps in care and recommend actionable solutions to improve health outcomes
  • Focus solely on assessmentsno prescribing medication or developing treatment plans required

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board Certification via ANCC or AANP
  • Current BLS/CPR certification
  • MSN from an accredited nursing program
  • A minimum of six months recent experience working as an NP in home health (preferred)
