Take advantage of a highly flexible per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This role involves conducting comprehensive home visits throughout Calhoun County, FL, including Blountstown, Altha, and neighboring areas. Each home health risk assessment generally takes about 45 minutes, focusing on Medicare Advantage members. Depending on the scheduling needs, you may complete annual health assessments or post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Option to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual risk assessments and post-discharge visits in home settings for Medicare Advantage plan members
- Review and document previous and current medical history, health conditions, and medication usage
- Educate individuals on health conditions and their medications
- Detect potential gaps in care and offer recommendations for follow-up
- No prescribing or treatment plan creation necessary
Qualifications:
- Active Nurse Practitioner license in Florida
- MSN from an accredited nursing program
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- Minimum of 6 months NP experience preferred
