Job Description

Deliver advanced care where it matters mostright in patients homes. This per diem nurse practitioner opportunity focuses on conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout Hartford County, CT. Youll have the autonomy to craft your ideal schedule, working flexible shifts any day of the week, and make a direct impact by assessing members in Hartford, New Britain, West Hartford, and surrounding areas. Each evaluation averages 45 minutes, ranging from annual assessments to post-discharge visits.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Build your own schedule based on availability
  • Set your minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Complete annual and post-discharge home health risk assessments
  • Review and assess patients prior and current medical histories and medications
  • Deliver education tailored to members regarding conditions and medication regimens
  • Identify care gaps, offering recommendations to support improved health outcomes
  • Note: This role involves evaluation and education onlyno treatment planning or prescribing

Qualifications:

  • Active Nurse Practitioner license in Connecticut
  • Current CPR (BLS) certification
  • Board certification from AANP or ANCC
  • Minimum six months of recent experience as an NP preferred
  • MSN degree from an accredited nursing program

This position offers competitive compensation of $80-$90 for each completed evaluation, plus a mileage stipend and compensation for administrative time.
