Job Description

Make a meaningful impact in your community by performing comprehensive home health risk assessments for Medicare Advantage plan members. This per diem opportunity offers dynamic, flexible shifts and empowers you to create your own scheduleproviding coverage throughout Monroe County, including Brinkley, Clarendon, and Holly Grove. Youll play an integral role in supporting patient health outcomes and providing valuable education during annual or post-discharge home visits.

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to accommodate your life
  • Minimum required commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct in-home health risk assessments for Medicare Advantage members
  • Review and document current and past medical history, including medication and conditions
  • Educate patients regarding their health conditions, medications, and wellness needs
  • Identify gaps in care and offer recommendations to address them
  • Visits last approximately 45 minutes; no prescribing or treatment plan creation required

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current CPR certification (BLS required)
  • Board Certified Nurse Practitioner (AANP or ANCC)
  • Master of Science in Nursing from an accredited program
  • At least 6 months of recent experience practicing as an NP preferred
