Schedule:

• Flexible shifts available Monday through Sunday
• Choose shifts that suit your lifestyle
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct thorough annual home health risk assessments for Medicare Advantage plan members
• Evaluate patient's prior and current medical history, medical conditions, and medication use
• Offer education to patients regarding their medical conditions and medications
• Identify and document gaps in care, providing clear recommendations for follow-up
• Note: No responsibilities to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in West Virginia
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• Graduation from an accredited MSN program
• At least 6 months of current experience as a Nurse Practitioner preferred
