Step into a rewarding Nurse Practitioner opportunity offering flexible per diem shifts in home health care. This role focuses on performing risk assessments for Medicare Advantage plan members within New Haven County, CTincluding residents of New Haven, Waterbury, Hamden, and surrounding areas. Each home health risk assessment typically lasts around 45 minutes and may include annual health evaluations or post-discharge check-ins.

Schedule:

• Choose your own shifts with flexibility between Monday and Sunday
• Design your weekly or bi-weekly schedule to fit your needs
• Minimum hourly commitment required

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members in their homes
• Review both prior and current medical history, including conditions and medications
• Educate patients on disease management and medication use
• Detect care gaps and advise on recommended follow-up steps
• Note: No prescription writing or treatment planning required

Qualifications:

• Current Nurse Practitioner license in Connecticut
• Active CPR certification (BLS)
• Board certification through AANP or ANCC
• MSN degree obtained from an accredited nursing institution
• At least 6 months of recent NP experience preferred

In this position, youll receive $80$90 for each evaluation completed, along with a mileage stipend and compensation for administrative time.
