Make a meaningful difference in community health as a Nurse Practitioner specializing in home health risk assessments. This per diem position offers exceptional flexibility and an opportunity to utilize your advanced clinical expertise to improve patient outcomes for Medicare Advantage members across New Haven County.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly commitment required
Primary Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Assess medical history, ongoing conditions, and medication regimens
- Educate patients and caregivers on health conditions and medication management
- Detect gaps in care and offer actionable recommendations for follow-up
- Strictly perform evaluationsno treatment planning or prescription responsibilities
Desired Qualifications:
- Active Nurse Practitioner License in Connecticut
- Current BLS/CPR certification
- Board Certification via AANP or ANCC
- At least 6 months recent experience as an NP (preferred)
- MSN degree from an accredited nursing program
