Job Description

Make a meaningful impact in the lives of Medicare Advantage plan members by providing essential home health risk assessments throughout New Haven County. This per diem nurse practitioner opportunity offers the flexibility to create your own schedule with shifts available any day of the week, Monday through Sunday, and requires a minimum weekly or bi-weekly hourly commitment. Youll have autonomy in crafting a work-life balance that fits your needs while serving communities across New Haven, Waterbury, Hamden, and other locations within the county.

Schedule:

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments, averaging 45 minutes each
  • Review and evaluate medical history, current conditions, and medications
  • Educate patients on health conditions and medication usage
  • Identify and document care gaps, recommending solutions for resolution
  • Focus exclusively on assessmentsno treatment planning or prescribing required

Qualifications:

  • Active Nurse Practitioner license in Connecticut
  • Board Certification via AANP or ANCC
  • Current CPR (BLS) certification
  • Master of Science in Nursing (MSN) degree from an accredited school
  • At least 6 months of recent NP experience preferred

This role provides competitive compensation of $80$90 per evaluation, a mileage stipend, and payment for administrative time, ensuring your contributions are recognized and rewarded.
