Take on a rewarding per diem opportunity working as a Nurse Practitioner conducting home health risk assessments throughout Fairfield County, CT. This role involves supporting Medicare Advantage plan members residing in the Bridgeport area and surrounding locations such as Stamford and Norwalk. Schedule:

  • Flexible shifts available between Monday and Sunday
  • Choose your preferred working days
  • Minimum weekly or bi-weekly hourly commitment, tailored to your availability

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage health plan members
  • Assess prior and current medical history, chronic conditions, and medication use
  • Offer education regarding medical conditions and prescribed medications
  • Recognize care gaps and provide recommendations to improve patient outcomes
  • No need to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in the state of Connecticut
  • CPR certification (BLS) is required
  • Board certified through AANP or ANCC
  • At least 6 months of recent NP experience is preferred
  • MSN degree from an accredited nursing program
