Schedule:

  • Flexible shifts between Monday and Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review prior and current medical histories, along with medication profiles
  • Offer education regarding medical conditions and proper medication use
  • Identify gaps in care and share recommendations to address them
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Connecticut
  • Current CPR (BLS) certification
  • Board Certification from AANP or ANCC
  • At least 6 months of current Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing school

In return for your valued expertise and compassionate care, you will earn $80 to $90 per evaluation, along with a mileage stipend and compensation for administrative time.
