This rewarding per diem opportunity focuses on patients across Bridgeport and throughout Fairfield County, including areas such as Stamford and Norwalk. Schedule:
- Flexible shifts between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review prior and current medical histories, along with medication profiles
- Offer education regarding medical conditions and proper medication use
- Identify gaps in care and share recommendations to address them
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Connecticut
- Current CPR (BLS) certification
- Board Certification from AANP or ANCC
- At least 6 months of current Nurse Practitioner experience preferred
- MSN degree from an accredited nursing school
