This opportunity is ideal for Nurse Practitioners passionate about delivering personalized care in a home setting. Per diem positions are available, offering flexibility and the unique chance to conduct in-home health risk assessments for Medicare Advantage plan members throughout various areas in Ocean County, NJ.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule to accommodate personal and professional needs
• Minimum weekly or bi-weekly hourly commitment required

As a key member of the home health team, you will:

• Perform comprehensive annual risk assessments in patients homes, taking approximately 45 minutes per visit
• Review and evaluate medical history, current health conditions, and medication regimens
• Provide patient education related to chronic conditions, medications, and preventive care
• Identify care gaps and offer recommendations to improve overall health outcomes
• Visits may include annual assessments or post-discharge check-insno requirement for writing treatment plans or prescribing medications

Core Qualifications:

• Active Nurse Practitioner (NP) License in New Jersey
• Board certification through AANP or ANCC
• Current BLS/CPR certification
• MSN from an accredited nursing school
• At least 6 months of recent, relevant NP experience preferred
