Advance your clinical career as a Nurse Practitioner conducting comprehensive home health risk assessments for Medicare Advantage members across Ocean County, NJincluding Lakewood, Toms River, and Brick. This per diem opportunity is ideal for professionals seeking autonomy and flexibility.
Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Commitment options: minimum weekly or bi-weekly hours
Key Responsibilities:
- Perform annual and post-discharge risk assessments in patients homes, evaluating medical history, chronic conditions, and current medications
- Deliver education to members regarding medical conditions and proper medication use
- Identify potential care gaps and offer actionable recommendations to help address them
- No requirement for prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current BLS (CPR) certification
- At least 6 months of recent nurse practitioner experience preferred
- Proficiency in patient education and documentation
