Schedule/Hours:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Commitment options: minimum weekly or bi-weekly hours

Key Responsibilities:

• Perform annual and post-discharge risk assessments in patients homes, evaluating medical history, chronic conditions, and current medications
• Deliver education to members regarding medical conditions and proper medication use
• Identify potential care gaps and offer actionable recommendations to help address them
• No requirement for prescribing medications or developing treatment plans

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Board Certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• Current BLS (CPR) certification
• At least 6 months of recent nurse practitioner experience preferred
• Proficiency in patient education and documentation
