Take on a vital role as a Nurse Practitioner in the home health sector, providing direct patient care and conducting risk assessments for Medicare Advantage members. This per diem position offers flexibility and the ability to tailor your schedule to your preferences, with available shifts any day of the week. Youll visit patients in Brevard and surrounding areas within Transylvania County, offering crucial services while ensuring high-quality health outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Evaluate patients prior and current medical history, medical conditions, and medication usage
- Provide patient education regarding medical conditions and medication management
- Identify gaps in care and offer recommendations to close those gaps
- Document all assessments thoroughly; no prescribing or treatment plan creation is required
Qualifications:
- Valid Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited school of nursing
- At least 6 months of experience as an NP is highly preferred
