Job Description

Advance your clinical expertise as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity allows for flexibility and independence, as you deliver high-quality care to Medicare Advantage plan members throughout Williamson County, including Franklin, Brentwood, and Spring Hill. Youll play a vital role in supporting positive patient outcomes, assessing individual needs within the comfort of their own homes.

Schedule:

  • Choose your own shifts between Monday and Sunday
  • Set your own schedule to fit your availability
  • Satisfy minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct comprehensive annual and post-discharge health risk assessments in patient homes
  • Review and document medical history, current conditions, and medications
  • Educate members about their diagnoses and medication regimens
  • Recognize gaps in care and recommend appropriate actions to improve health outcomes
  • No need to create treatment plans or prescribe medicationsfocus solely on patient assessments

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Board certified through AANP or ANCC
  • Current BLS (CPR) certification
  • At least 6 months of recent NP experience preferred
  • MSN from an accredited nursing program
