Discover a highly flexible per diem role designed for experienced Nurse Practitioners seeking autonomy in their schedules. In this rewarding position, you will conduct home health risk assessments for Medicare Advantage plan members within Kitsap County, offering the opportunity to engage with patients in Bremerton and surrounding locales including Bainbridge Island, Silverdale, and Port Orchard.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Self-schedulingselect shifts that fit your life
• Minimum hourly commitment on a weekly or bi-weekly basis

Responsibilities:

• Perform comprehensive annual and post-discharge health risk assessments in members homes
• Obtain and evaluate prior and current health histories, existing medical conditions, and medication usage
• Educate patients about their medical conditions and medications, supporting improved health literacy
• Identify gaps in care and offer actionable recommendations to close them
• No requirement to establish treatment plans or prescribe medications

Preferred Qualifications:

• Active Nurse Practitioner License issued by the state of WA
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• At least 6 months of current NP experience (highly preferred)
