Job Description
Make a meaningful impact delivering in-home care as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity allows you to provide essential healthcare services to Medicare Advantage members throughout Vigo County, IN, including Terre Haute, West Terre Haute, and Seelyville.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members in their homes
- Assess prior and current medical and medication histories
- Evaluate current health status and chronic conditions
- Provide patient education on medical issues and medication management
- Identify gaps in care, offering actionable recommendations to optimize health outcomes
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- MSN degree from an accredited nursing school
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- Minimum 6 months of recent NP experience preferred
- Previous home health experience is an asset
