Advance your clinical expertise as a Nurse Practitioner focused on home health risk assessments in the Brazil, IN area. This per diem opportunity is perfect for experienced providers seeking flexible scheduling and the chance to make a direct impact on patient health in a community setting. Youll work with Medicare Advantage members in Parke County, including locations such as Rockville, Montezuma, and Marshall. Each assessment, ranging from annual health reviews to post-discharge visits, typically lasts around 45 minutes and is tailored to the individual needs of the patient.
Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own schedule to fit your personal and professional needs
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage plan members
- Review prior and current medical histories, medical conditions, and medication use
- Educate patients on their health conditions and proper medication management
- Identify care gaps and offer actionable recommendations for care improvement
- Focus exclusively on assessmentno need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- MSN degree from an accredited nursing school
Compensation is $80-$90 per evaluation, complemented by a mileage stipend and administrative pay for your professional services and travel.
