Job Description

Step into a dynamic per diem opportunity as an experienced Nurse Practitioner providing home health risk assessments throughout Clay County, Indiana. This role offers you the autonomy to craft your own schedule and deliver meaningful care to Medicare Advantage plan members in the local community.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct comprehensive annual health risk assessments for Medicare Advantage members, including post-discharge and annual evaluations
  • Travel to patients homes across Brazil, Clay City, Knightsville, and the wider Clay County area
  • Review and evaluate patient medical history, current conditions, and medication usage
  • Educate patients on their conditions and prescribed medications to empower informed health decisions
  • Identify care gaps and offer recommendations to help close those gaps for improved outcomes
  • No responsibilities related to ongoing treatment planning or prescribing

Qualifications:

  • Valid Nurse Practitioner license in Indiana
  • Board Certification via AANP or ANCC
  • MSN degree from an accredited nursing program
  • Current BLS (CPR) certification
  • At least 6 months experience as a Nurse Practitioner is preferred
