Deliver high-quality, patient-centered care as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members throughout Woodruff County, AR. This per diem role offers the flexibility to create your own schedule, making it an excellent fit for providers seeking autonomy and the opportunity to make a meaningful impact in their community.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own schedule
- Maintain a minimum hourly commitment per week or bi-weekly
Key Responsibilities:
- Perform comprehensive annual health risk assessments and post-discharge visits in patient homes
- Evaluate patients prior and current medical history, ongoing health conditions, and medication use
- Educate patients about their diagnoses, medications, and wellness strategies
- Identify care gaps and recommend appropriate steps to address them
- Visits last approximately 45 minutes; no requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Arkansas (AR)
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of NP experience is preferred
