Job Description

Deliver essential care and perform impactful assessments as a Nurse Practitioner specializing in home health risk assessments. This per diem role allows you to provide clinical expertise and education to Medicare Advantage plan members in various communities throughout Jackson County, AR.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create a personalized work schedule
  • Choose between minimum weekly or bi-weekly hourly commitments

Primary Responsibilities:

  • Conduct comprehensive annual and post-discharge home health risk assessments during 45-minute in-home visits
  • Travel to patient homes in Newport, Tuckerman, Diaz, and other areas within Jackson County
  • Review and evaluate each patient's prior and current medical history, chronic conditions, and medication regimen
  • Educate patients about their health conditions and prescribed medications, empowering them to participate actively in their well-being
  • Detect gaps in care and communicate actionable recommendations to improve patient outcomes
  • Focus solely on assessmentsno development of treatment plans or prescribing medication required

Requirements:

  • Active, unrestricted Nurse Practitioner license in Arkansas (AR)
  • Current CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • Minimum of 6 months of experience as a Nurse Practitioner preferred
  • MSN degree earned from an accredited nursing program

This opportunity offers competitive compensation of $80-$90 per completed evaluation, with an added mileage stipend and administrative time included.
