Deliver essential care to individuals in their homes as a Nurse Practitioner conducting home health risk assessments. This valuable per diem opportunity allows providers to serve Medicare Advantage members across Berks County, ensuring thorough annual assessments and patient education in a flexible, autonomous work environment.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose your preferred weekly or bi-weekly hour commitment
Key Responsibilities:
- Complete comprehensive home health risk assessments, averaging 45 minutes per visit
- Review medical history, current health status, and medication usage with patients
- Offer detailed education regarding diagnoses, conditions, and medications
- Identify care gaps and recommend actionable next steps to close those gaps
- Focus visits on assessment and education; no need to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Pennsylvania
- Board Certification through AANP or ANCC required
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
