This per diem opportunity allows you to create your own schedule, with flexible shifts available any day of the week. Meet with Medicare Advantage plan members in Lynn, Lawrence, Haverhill, and nearby locations to assess health needs in the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Build your own schedule based on your availability
- Commit to a minimum number of weekly or bi-weekly hours
Core Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior and current medical history, conditions, and medication usage
- Offer patient education regarding diagnoses, ongoing conditions, and prescribed medications
- Detect gaps in care and provide actionable recommendations for the health team
- Visits may include routine annual health assessments or focused post-discharge assessments
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Massachusetts
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- Six months of recent nurse practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
