Job Description

Make an impact in community health by conducting in-home risk assessments as a Nurse Practitioner focusing on Medicare Advantage members in Suffolk County, Massachusetts. This per diem, home health opportunity allows you to create your own schedule, providing flexibility while supporting patient care in Boston and surrounding areas such as Revere and Chelsea.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own weekly or bi-weekly hours
  • Choose shifts that fit your lifestyle and availability

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members within Suffolk County
  • Review and document prior/current medical histories, medical conditions, and medication usage
  • Educate patients about their medical conditions and prescribed medications
  • Recognize care gaps and recommend actionable solutions
  • Visits may include annual assessments or post-discharge follow-ups
  • No treatment plan development or medication prescribing required

Qualifications:

  • Current and active Nurse Practitioner license in Massachusetts
  • Board Certification via AANP or ANCC
  • Valid CPR certification (BLS required)
  • Minimum of six months NP experience strongly preferred
  • Master of Science in Nursing (MSN) from an accredited institution

As part of this role, you will receive compensation of $80-$90 per evaluation, in addition to a mileage stipend and pay for administrative time. This position is designed for NPs seeking flexibility and autonomy while providing direct patient care in a home health setting.
