Take your advanced practice skills into the community by providing essential healthcare assessments for Medicare Advantage members through home health visits. As a Nurse Practitioner specializing in Home Health risk assessments, you'll visit patients in Middlesex Countyincluding Cambridge, Lowell, and Newtonhelping to close care gaps and deliver high-quality evaluations.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your personal commitments
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive home health risk assessments (approx. 45 minutes per visit) for Medicare Advantage plan members
- Review and evaluate patients prior and current medical histories, existing conditions, and medication usage
- Deliver patient education on medical conditions and proper medication use
- Identify gaps in care and advise on next steps for closing those gaps
- Focus exclusively on assessmentsno treatment plans or prescribing necessary
Qualifications:
- Active Nurse Practitioner license in Massachusetts
- Current CPR certification (BLS required)
- AANP or ANCC board certification as a Nurse Practitioner
- MSN degree from a recognized, accredited nursing program
- Minimum of six months current Nurse Practitioner experience preferred
