Take on a vital healthcare role by conducting home health risk assessments for Medicare Advantage plan members throughout Essex County. This per diem opportunity offers Nurse Practitioners the flexibility to create their own schedules with shifts available any day of the week, making it ideal for those seeking work-life balance. With a focus on delivering high-quality patient care, practitioners will perform comprehensive annual assessments and post-discharge visits within patients homes, primarily in areas such as Lynn, Lawrence, Haverhill, and nearby locations.
Schedule:
- Flexible shifts available Monday through Sunday
- Option to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Review and document prior and current medical history, conditions, and medication regimens
- Offer education to patients regarding their health conditions and prescribed medications
- Identify care gaps and recommend strategies for closing them
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Massachusetts
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- At least 6 months of recent experience working as an NP (preferred)
- MSN degree from an accredited nursing program
