Explore an impactful opportunity to provide high-quality home health risk assessments as a Nurse Practitioner. This per diem position offers professionals the autonomy to create their own schedules while serving Medicare Advantage plan members throughout Hutchinson Countyincluding Borger, Stinnett, and Fritchby delivering essential in-home care evaluations and education.

Schedule:

• Flexible shifts available between Monday and Sunday
• Choose your own hours and days each week
• Meet a minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Perform annual and post-discharge home health risk assessments for Medicare Advantage members
• Assess and review patients prior and current medical histories, medical conditions, and medication history
• Educate patients on their medical conditions and medication management
• Identify gaps in care and make actionable recommendations for resolution
• No treatment plan development or medication prescribing required
• Each assessment typically takes about 45 minutes

Qualifications:

• Active Nurse Practitioner license in Texas
• Current BLS/CPR certification
• Board Certification through AANP or ANCC
• MSN degree from an accredited program
• Minimum of 6 months experience as a practicing NP preferred

This position recognizes your expertise and commitment by offering $80-$90 per completed evaluation, along with a mileage stipend and additional compensation for administrative time.
