Enjoy the autonomy and flexibility of a per diem Nurse Practitioner role focused on delivering high-quality care through home health risk assessments across Surry County, NC. As a key member of an innovative healthcare team, youll impact patient outcomes by providing detailed assessments to Medicare Advantage members in Mount Airy, Elkin, Dobson, and surrounding areas.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment needed
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage health plan members
- Evaluate patients prior and current medical history, health status, and medication usage
- Educate patients on their medical conditions and medications to support long-term wellness
- Identify and document gaps in care, providing actionable recommendations for care improvement
- No need to develop treatment plans or prescribe medicationsfocus on assessment and care coordination
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Current BLS/CPR certification
- Board certified through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of NP experience preferred
Professionals in this role are compensated at $80-$90 per completed evaluation, including a mileage stipend and administrative time.
