Enjoy the flexibility of per diem work as a Nurse Practitioner specializing in home health risk assessments. This unique opportunity allows you to deliver essential care to Medicare Advantage members throughout Avery County, including communities such as Banner Elk, Beech Mountain, Crossnore, and Elk Park. Home visits focus on annual health assessments or post-discharge evaluations, each lasting around 45 minutes. Designed for self-motivated clinicians, this role combines autonomy with community impact.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Evaluate previous and current medical history, chronic conditions, and medication usage
- Educate patients about their health conditions and medications
- Identify gaps in care and provide actionable recommendations for improvement
- No duties involving creation of treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- MSN degree from an accredited nursing program
- Current CPR certification (BLS)
- Board certification via AANP or ANCC
- A minimum of 6 months current NP experience preferred
