Job Description

Make a real impact in patient care with this per diem opportunity to perform home health risk assessments across Collier County, FL. As a Nurse Practitioner specializing in home health, you will conduct comprehensive risk assessments for Medicare Advantage plan members. Each evaluation typically takes about 45 minutes and may focus on annual health assessments or post-discharge visits, with assignments in Naples, Immokalee, Marco Island, and surrounding locations.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Freedom to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage health plan members
  • Review and document prior and current medical history, existing medical conditions, and ongoing therapies
  • Educate patients regarding their conditions and medications, fostering better health literacy and engagement
  • Identify potential care gaps; provide clinical recommendations for follow-up as needed
  • No need for treatment plan development or prescribing medicationsfocus on assessments and education

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Current CPR certification (BLS)
  • Board Certified through AANP or ANCC
  • At least 6 months of current NP experience preferred
  • MSN degree from an accredited nursing program
