Step into a highly independent, patient-centered role performing home health risk assessments for Medicare Advantage plan members in Shawano County, WI, including Shawano, Bonduel, and Birnamwood. As a Nurse Practitioner, you will leverage your clinical expertise to assess patients in their homes, ensuring they receive comprehensive, compassionate care and critical health evaluations.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Set your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform in-home annual health risk assessments that typically last 45 minutes per visit
• Review prior and current medical histories, medical conditions, and medication histories thoroughly
• Offer education about medical conditions and medication regimens to patients and families
• Identify potential care gaps, recommend solutions, and help coordinate timely follow-up
• Focus exclusively on assessmentsno treatment planning or medication prescribing required

Qualifications:

• Current Nurse Practitioner license in Wisconsin
• Active CPR certification (BLS)
• Board certification through AANP or ANCC
• At least 6 months of current experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing program
