Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members in various communities within Allegany County
• Review and evaluate medical and medication histories, as well as previous and current health conditions
• Offer education related to medical diagnoses and medication management
• Identify care gaps and recommend necessary next steps to help members achieve optimal health outcomes
• Complete visits in approximately 45 minutes; services may involve routine assessments or post-discharge visits
• No need to create treatment plans or prescribe medications

Desired Qualifications:

• Active Nurse Practitioner license in the state of New York
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• Minimum of six months recent experience as a Nurse Practitioner (preferred)
• Commitment to providing compassionate, patient-centered care in a home-based setting

This role offers compensation of $80 to $90 per evaluation, complemented by a mileage stipend and additional administrative time.
