Job Description
Deliver essential healthcare services and make a direct impact in the community by conducting in-home risk assessments for Medicare Advantage plan members residing in a variety of locations within Allegany County, NY. As a Nurse Practitioner specializing in home health, you will provide comprehensive assessments to support patient well-being, with a focus on preventive care and patient education.

- Commit to a minimum number of hours weekly or bi-weekly, tailored to your preferences
Primary Responsibilities:
- Perform annual home health risk assessments in patients residences, each typically lasting about 45 minutes
- Review and evaluate prior/current medical histories, ongoing medical conditions, and medication regimens
- Offer education to patients regarding their health conditions and medications, empowering them in their care
- Identify possible gaps in patient care and recommend solutions
- Visits may include annual assessments or follow-ups after a patients discharge
- No treatment plan development or prescribing required
Qualifications:
- Active Nurse Practitioner license in New York State
- Current CPR certification (BLS)
- Board Certified through AANP or ANCC
- Preferably at least six months of current NP experience
- MSN from an accredited nursing program
