Job Description

Deliver essential healthcare services and make a direct impact in the community by conducting in-home risk assessments for Medicare Advantage plan members residing in a variety of locations within Allegany County, NY. As a Nurse Practitioner specializing in home health, you will provide comprehensive assessments to support patient well-being, with a focus on preventive care and patient education.

  • Commit to a minimum number of hours weekly or bi-weekly, tailored to your preferences

Primary Responsibilities:

  • Perform annual home health risk assessments in patients residences, each typically lasting about 45 minutes
  • Review and evaluate prior/current medical histories, ongoing medical conditions, and medication regimens
  • Offer education to patients regarding their health conditions and medications, empowering them in their care
  • Identify possible gaps in patient care and recommend solutions
  • Visits may include annual assessments or follow-ups after a patients discharge
  • No treatment plan development or prescribing required

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Current CPR certification (BLS)
  • Board Certified through AANP or ANCC
  • Preferably at least six months of current NP experience
  • MSN from an accredited nursing program
