Take the next step in your advanced practice career by providing critical healthcare services directly to patients in their homes. This per diem opportunity empowers Nurse Practitioners to conduct home health risk assessments for Medicare Advantage members throughout Allegany County, NY. Assessment visits average 45 minutes and could include annual health evaluations or post-discharge check-ins. Coverage may be assigned in Wellsville, Alfred, Cuba, Caneadea, and other communities within Allegany County.
Schedule:
- Flexible shifts, offered Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage plan members
- Review and document current/prior medical history, medical conditions, and medications
- Deliver patient education related to health conditions and prescribed medications
- Identify care gaps and provide actionable recommendations to optimize health outcomes
- This position focuses solely on assessment and educationno required prescribing or treatment plan creation
Qualifications:
- Active Nurse Practitioner license in New York
- Master of Science in Nursing (MSN) from an accredited institution
- Board Certified through AANP or ANCC
- Current Basic Life Support (BLS) CPR certification
- At least 6 months' experience as a Nurse Practitioner preferred
