This per diem nurse practitioner position offers the autonomy to create your own schedule, with flexible shifts available any day of the week and no minimum hourly commitment required.

Key Responsibilities:

  • Conduct annual and post-discharge risk assessments in patients homes for a Medicare Advantage health plan
  • Review and document comprehensive medical histories, chronic conditions, and medication lists
  • Deliver education to patients regarding their health conditions and prescribed medications
  • Identify care gaps and make clinical recommendations to improve outcomes
  • No creation of treatment plans or prescribing medications required

Desired Qualifications:

  • Current Nurse Practitioner license in Louisiana
  • Active Board Certification (AANP or ANCC)
  • MSN from an accredited nursing program
  • Current Basic Life Support (BLS) CPR certification
  • At least 6 months of nurse practitioner experience preferred
