Job Description
Take on an impactful role conducting home health risk assessments for Medicare Advantage members in Broward County. As a Nurse Practitioner specializing in home health, youll have the autonomy of per diem work with flexible schedulingshifts are available any day of the week and you can tailor your weekly or biweekly commitment to fit your needs. Assignments may be in various Broward County locations, including Fort Lauderdale, Pembroke Pines, and Hollywood.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-scheduling options
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual risk assessments and post-discharge visits in members' homes
- Review patients medical histories, current conditions, and medication regimens
- Educate patients about their health conditions and medications
- Identify gaps in care and make recommendations to improve patient health outcomes
- No requirement for creating detailed treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in Florida
- Current BLS (CPR) certification
- Board certification via AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
