Job Description

Make a meaningful impact on patient care by conducting home health risk assessments for Medicare Advantage members throughout Adams County, IN. This per diem opportunity allows Nurse Practitioners to utilize their clinical skills in a flexible, patient-centered capacity, focusing on proactive health evaluations without the need to prescribe or manage ongoing treatment plans.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Choose minimum weekly or bi-weekly hour blocks to match your availability

Key Responsibilities:

  • Perform comprehensive, in-home annual or post-discharge risk assessments (approx. 45 minutes per visit)
  • Review and evaluate prior/current medical history, conditions, and medication usage
  • Educate patients about their medical conditions and prescribed medications
  • Identify any gaps in care and recommend actions or resources to address them
  • Maintain accurate health records in accordance with compliance and company standards

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Board Certified through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN from an accredited school of nursing
  • At least six months of recent Nurse Practitioner experience is preferred

This role offers competitive compensation, including $80-$90 per completed evaluation, with an added mileage stipend and payment for administrative time.
