Job Description
Deliver vital care within a supportive, patient-centered home health environment as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity is focused on Medicare Advantage members across Cibola County, NMincluding the communities of Grants, Milan, and Acoma Pueblo. You'll play a crucial role in care continuity by performing in-depth annual or post-discharge health assessments during home visits.
Schedule
- Flexible shifts available Monday through Sunday
- Self-scheduling to match your availability
- Minimum weekly or bi-weekly hour commitment
Key Responsibilities
- Complete comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior and current medical history, including all medical conditions and medication regimens
- Educate patients regarding their diagnoses and medication management
- Identify care gaps and recommend steps for closure
- No development of treatment plans or medication prescribing responsibilities
Qualifications
- Active Nurse Practitioner license in New Mexico
- Current CPR certification (BLS)
- Board Certification from AANP or ANCC
- Completion of MSN from an accredited nursing school
- Minimum 6 months of nurse practitioner experience preferred
Compensation
