This per diem opportunity puts your advanced clinical skills to work by supporting Medicare Advantage plan members throughout Kanawha County, including Bluefield, Princeton, and Athens. You'll appreciate a highly flexible schedule in which you choose your shifts between Monday and Sunday, with the option to create a timetable that suits your needs and determine your own minimum weekly or bi-weekly hourly commitment.

Schedule:

• Flexible shifts available Monday to Sunday
• Design and manage your own work schedule

Key Responsibilities:

• Conduct in-home annual health risk assessments for Medicare Advantage members
• Assess medical histories, ongoing conditions, and medication usage
• Educate patients on health conditions and medication adherence
• Identify care gaps and provide actionable recommendations
• No requirement for creating treatment plans or prescribing medications
• Each assessment lasts approximately 45 minutes

Qualifications:

• Active Nurse Practitioner license in West Virginia
• Board Certification (AANP or ANCC)
• Current BLS CPR certification
• Six months of recent NP experience preferred
• MSN degree from an accredited nursing school
