Bring your expertise as a Nurse Practitioner to a dynamic, patient-focused opportunity conducting home health risk assessments throughout Avery County, NC. This per diem position offers great autonomy, enabling you to craft your own schedule and work flexible hours. Youll visit Medicare Advantage plan members in their homesvisits may include annual health assessments or post-discharge evaluations, each averaging around 45 minutes. Service areas include Banner Elk, Beech Mountain, Crossnore, Elk Park, and other locations within Avery County.
Schedule:
- Enjoy total scheduling flexibility between Monday and Sunday
- Set your own shift times
- Choose your preferred minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate both past and present medical history, ongoing conditions, and current medication use
- Educate patients on their diagnoses, condition management, and prescribed medications
- Identify gaps in care and provide actionable recommendations to enhance patient outcomes
- No requirement for treatment plan development or prescribing medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification via ANCC or AANP
- MSN degree from an accredited nursing program
- Current BLS/CPR certification
- At least 6 months of Nurse Practitioner experience preferred
