This role involves conducting home health risk assessments for Medicare Advantage members residing in Blountstown and throughout Calhoun County. You will engage with patients in a variety of home settings, providing crucial evaluations that inform ongoing wellness and care.
Schedule:
- Choose flexible shifts between Monday and Sunday
- Design your own work schedule
- Meet minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review and assess patients medical history, chronic conditions, and current medications
- Deliver education on disease processes and medication adherence
- Identify gaps in care and suggest appropriate follow-up or interventions
- Focus solely on assessmentno prescribing or development of treatment plans required
Qualifications:
- Current, valid Nurse Practitioner license in the state of FL
- CPR certification (BLS) required
- Board certification through AANP or ANCC
- At least 6 months of recent NP experience preferred
- Graduation from an accredited MSN program
For each assessment, you'll be compensated $80$90, with additional stipends for mileage and administrative documentation time. This structure supports a balanced approach to providing care in a home environment, tailored around your availability.
