Advance your career as an independent Nurse Practitioner performing meaningful work through home health risk assessments within Lamar County, TX. This per diem opportunity offers the flexibility to create your own schedule, working with Medicare Advantage plan members in communities such as Blossom, Reno, Powderly, Roxton, and Paris, TX.
Schedule:
- Flexible shifts offered Monday through Sunday
- Build your own schedule around your availability
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Complete comprehensive annual or post-discharge health risk assessments in patients' homes
- Evaluate and document prior and current medical history, existing conditions, and medication use
- Educate patients on medical conditions and medication management
- Identify gaps in care and provide clear recommendations for follow-up or resolution
- No requirement to create ongoing treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Texas
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- MSN from an accredited nursing school
- Minimum of 6 months' current experience as a Nurse Practitioner preferred
